Our Mission at 5 Star Home Health and Hospice is to provide the highest quality home health care and Hospice possible in the most professional and ethical manner. We are dedicated to promoting the physical and emotional well-being of our patients and all who come in contact with our Agency. What We Do
Home Health: We have an extensive team of therapists to help manage your care and recovery at home including physical therapists, occupational therapists, and speech therapists. If you or your loved one is having any of the following problems our home health therapists can help
Hospice: The whole family is considered the unit of care, and care extends through their period of mourning. Hospice services can be provided wherever a patient calls home: private residence, assisted living center, skilled nursing facility, etc. Services are typically related to managing the patient's terminal illness, controlling pain, or enabling the individual to maintain activities of daily living and basic functional skills.
